Complete the forms, checking that you have included your
- NPD number (in the handbook after your name)
- Correct postcode
(there is no need to enter your full address)
NB: Multi Caution Match Report Forms
Ensure all cautions in the game are entered - (for both teams) - and that you have completed the bottom box/boxes confirming which County or Counties you are submitting your form to (as applicable)
1) Save the completed form to your hard-drive memory using an easily identifiable title.
2) Start your email sending programme (e.g. msOutlook) and add the report as an attachment.
3) Email your completed forms to the County at which they are affiliated:-
Huntingdonshire FA teams to: discipline@huntsfa.com
Cambridgeshire FA teams to: discipline@cambridgeshirefa.com
When you submit your report, you will receive an automatic acknowledgement, by email, confirming that the Huntingdonshire FA has received your report/s.
No hard copy acknowledgement will be sent.
